Relay of the list of medical institutions accepting foreign patients compiled by the Ministry of Health, Labour and Welfare of Japan (MHLW) and the Japan Tourism Agency from the institutions selected by each prefecture, processed by MK Career Planning (not official). This is a public-service listing, not an enforcement or quality list: being listed is not an assessment of quality, and not being listed does not mean that an institution does not treat foreign patients (MHLW states that the list does not prevent other institutions from treating them). The list is not a list of institutions for people who travel to Japan for medical treatment. Languages are read from the language-support columns as published (they can include interpretation by phone, video or translation devices) and say nothing about who is available at a given time.
Statuses (foreign_patient_hospital_lookup, core registry-status vocabulary): listed (in the current list version, by exact name; or institutions match a prefecture query), no_longer_listed_since (seen by this service in an earlier version, not in the current one since the date given; the list does not state why; not an enforcement list), not_found_in_checked_records (nothing matches in the current version; this does not mean that institutions there do not treat foreign patients), unknown (list not loaded or stale: list_not_loaded_yet, stale; or only partial-name candidates: candidates_only).
Categories: the prefecture selects regional base institutions for foreign patients: category_1 (able to accept emergency patients who need hospitalization; at least one per prefecture) and category_2 (able to accept foreign patients, including clinics and dental clinics; at least one per secondary medical area). Other listed institutions were judged eligible by the prefecture without this selection (not_selected).
languages are ISO 639-1 codes read from the published language-support columns (departments and languages, dedicated department, medical coordinator, medical interpreter, remote interpretation, other support). The columns' own text (days, hours, vendors, notes) is not stored. A language listed there can mean interpretation by phone, video or a translation device, and the hours differ; call the institution to confirm before visiting.
Facility types: hospital (病院), clinic (診療所), dental_clinic (歯科診療所).
MHLW replaces the Excel file of the list (a new file URL each time) and does not keep earlier versions. This service records each list version it downloads (the page is checked about once a day; in 2026 the list was published on 2026-06-30 and partly changed on 2026-08-06 and 2026-09-30) and the differences per institution between versions, starting at tracking_since. An institution is one combination of prefecture and name as published (with the postal code when two institutions in a prefecture have the same name), so a renaming appears as one institution removed and another added. Changes are dated by when this service first downloaded the new version (observed_at). page_updated_on and page_update_note are MHLW's own dates on the page.
Free history window: change history recorded by this service is served for the last 30 days. A since earlier than that returns the last 30 days with history_window.clamped true (requested_since, served_since); older history can be requested as a paid product (not yet offered). tracking_since still shows when recording started.

## Payments

This server offers free tools only. It does not request or accept x402 payments.

## Rate limits

- Free calls (free HTTP endpoints under /v1/ and free MCP tools) are limited to 300 requests per 60 seconds per client (a client is keyed by a one-way hash of its IP address and the first 64 characters of its User-Agent; neither is stored. The User-Agent can be set by anyone, so this split is for fairness between clients behind a shared gateway, not a security control). Over the limit, HTTP returns 429 RATE_LIMITED with a Retry-After header and MCP tools return the error RATE_LIMITED. Paid calls are not rate limited.

## Provenance and fetch_status

Responses carry provenance[] (source, upstream_updated_at, retrieved_at, content_sha256, parser_version, fetch_status). fetch_status values:

- found: The upstream document was retrieved and contains the data used for this answer.
- empty: The upstream was retrieved, but it has no item for this query (for example JMA had no bulletin for that area in its feeds, or the municipality is not in the list). The state is unknown: empty does NOT mean 'none in effect'.
- stale: The data was retrieved but is older than the freshness threshold for its kind; treat it with care.
- unavailable: The upstream could not be retrieved, read or used (network error, withdrawn bulletin, paused source, or data not yet loaded).
- unsupported: The query is outside what this source covers (for example a date JMA does not issue warnings for yet, or a period before the source's records).

- RATE_LIMITED (free calls over the per-client limit; HTTP 429 with Retry-After; not charged)

## Terms

- Fresh: the MHLW page was checked successfully within 72 hours. Otherwise lookups return unknown and no institutions.
